Sessional Tenants Room Booking Change Request

If you require adhoc room booking changes please complete the request using the form below. Once your request has been actioned, you will be notified by email. If you have not received confirmation of the change to your booking, please contact the Summit Health Centre Landlord directly on shc.landlord@summithealth.org.au. Please note that we require 3 business days notice for any changes to bookings.

For longer term changes to your booking arrangements, please contact the Summit Health Centre Landlord directly on shc.landlord@summithealth.org.au as it is likely a variation to your service agreement/contract will be required. Do not use this form for longer term changes.
