  • Highly organised team player

  • Experience using medical practice software or a willingness to learn

About the role

This exciting and challenging role includes:

  • Working in collaboration with the GP to coordinate patient care by developing high-quality chronic condition management plans and team care arrangements, supporting patients in self-managing their chronic conditions.
  • Enabling patients to access multidisciplinary team care where needed.
  • Assist the GP by conducting health assessments for eligible patient groups to help in the early identification and prevention of chronic conditions.
  • Act proactively to engage the practice community in the prevention and self-management of chronic conditions.
  • Perform relevant clinical measurements and assessments and educate patients on health conditions and preventative measures.
  • Handle administrative tasks related to chronic condition management appointments, recalls, and reviews.
