Adelaide Hills Community Health Service is now offering a Chronic Condition Self Management Support Service funded through the Better Care in the Community (BCC) Program. BCC aims to reduce hospital admissions and length of hospital stay by improving the capacity of people in the community with chronic conditions to self manage optimally.

Examples may include patients with multiple co-morbitities who could benefit from goal setting and better service coordination.
