The Integrated Wellbeing Service (IWS) is funded by Country SA Primary Health Network and supports patients who require allied health or specialist nurse intervention to understand and manage their chronic disease conditions.
In partnership with FocusOne Health and Murray Mallee General Practice Network, Summit Health delivers the Integrated Wellbeing Service to Kangaroo Island.
Referrals must be made through a patient’s GP. A Care Coordinator will then assess an individual’s eligibility for services, based on their circumstances.
Referrals for Kangaroo Island patients are supported for these Allied Health Services:
- Nutrition and Dietetics Services
- Physiotherapy
- Illness Prevention / Lifestyle Coach
- Exercise Physiology
- Care Planning and Coordination
- Healthy Eating Programs
